Step 2: In-clinic Cosmelan 1 mask application. The skin is thoroughly degreased to ensure full penetration of the mask. Cosmelan 1 is applied evenly across the face in a clinical setting. The mask is left on for a period determined by Dr. Al Zaabi based on your Fitzpatrick skin type, pigmentation severity, and skin sensitivity. Lighter skin tones tolerate longer application times. Darker Fitzpatrick skin types require a shorter contact time to achieve depigmentation without inducing reactive hyperpigmentation.

Step 4: Cosmelan 2 maintenance. Starting 24 hours after mask removal, Cosmelan 2 cream is applied daily according to the schedule provided. Dermamelan: The Advanced Protocol

Dermamelan is Mesoestetic’s next-generation depigmentation system with a higher concentration formula and a more intensive home maintenance programme than Cosmelan. It is appropriate for patients with more severe or persistent pigmentation, those who have not achieved sufficient results from topical agents, or patients with deep, resistant melasma. The protocol structure is similar to Cosmelan but the active formula is more concentrated and the maintenance phase more intensive.

Dermamelan is not always the better option. What to Expect During the Treatment

The first 3 to 5 days after mask removal involve redness, tightness, and the beginning of skin peeling. The peeling phase lasts approximately 7 to 14 days. During this phase, skin sheds the pigmented outer layers and appears temporarily uneven. This is part of the mechanism and not a complication. It is important not to peel or pick the shedding skin.

By week 2 to 3, peeling resolves and the skin begins to show improved clarity and evenness. Significant improvement in pigmentation is visible by week 4. The full result develops over 2 to 3 months as the Cosmelan 2 maintenance phase continues to suppress melanin production and residual pigment fades.

Strict SPF 50 use with iron oxide is mandatory throughout the protocol and beyond. Any UV exposure during or after the protocol risks triggering rebound melanin production that can reverse the improvement.

Before the Treatment

Avoid sun exposure and tanning for 4 weeks before the mask session.
